Why This Spotlight
Real owners, real numbers, real challenges.
We are beginning to release a series of 'Owner Spotlights' to give you real insight into the experience, challenges, and insights taken from interviews with some really amazing small business owners like you.
We're all in this together and I hope you get some strength and encouragement from their stories.
Our first spotlight: Andrew Segarra has served his community for two decades, and like many practitioners he has navigated changing insurance dynamics, evolving patient needs, and the constant pressure of cash flow. His choices show what practical adaptation looks like.

- Twenty years in business, Andrew has grown his business to approximately five hundred thousand dollars in annual revenue.
- Pivoting away from an insurance dependent model as reimbursements and deductibles changed, expanding service lines to protect margins and improve outcomes.
- Differentiator, a broad toolkit beyond adjustments, including decompression, shockwave, class four laser, pulsation devices, trigonics, PEMF, and dry needling.
- Cash flow win, Andrew pays himself consistently even when times are tough.
- Current focus, scale the practice, grow revenue, refine marketing strategy.

Andrew’s Practice, two decades of care
Andrew has owned his chiropractic practice in North Carolina for twenty years. For years, his model leaned on health insurance. As reimbursement rates dropped and deductibles climbed, profitability suffered, and the old playbook stopped working.
Rather than wait it out, Andrew went deep on research. He broadened his clinical toolkit with spinal decompression, shockwave therapy, class four laser, pulsation devices, and soft tissue work like trigonics. He layered in magnetic field therapy, and he added dry needling, which is seeing strong demand. These options used to be reserved for specialized plans and aggressive injury treatments, such as knee replacement rehab.
Seeing an opportunity to add more value though, Andrew opened them up for all new patients, then tailored care to what produced the most effective and lasting relief. This transformed his business and set him apart from traditional Chiro practices.

What is working
- Patient experience that goes beyond a single technique, more paths to a win, more perceived value, stronger retention.
- Andrew learned early that paying himself consistently was vital - "Cut everything else first before losing owners pay".
- Clear positioning, a practice known for a range of advanced therapies, not just adjustments.
|
|
Where he is pushing next
- Scale the business while pursuing ownership of his physical location.
- Grow revenue with offers that bundle advanced therapies
- Rethink marketing beyond Facebook - test partnerships, talks in the community, local search, and patient referral systems.

Owner Playbook, Andrew’s advice to small business owners or aspiring entrepreneurs
- Entrepreneurship means taking risks. You'll have really great times, but you'll also get knocked down. Keep trying and don't give up.
- Get out of your comfort zone. Connect with people, build relationships, put your voice into the community and online.
- Protect owner pay. Pay yourself consistently, cut everything else first. "This is the number one rule in business."
- Keep it fun. Enjoy the work, your energy is part of the value customers feel.
